I probably got my example incorrect regarding the Grand Master of Assassins; it was probably a normal Custodian who refused to acknowledge him until the Inquisitorial Representative showed up. I just disliked the original statement of them being the highest rank in the Imperium, because "rank" does not equal authority or remit. The Custodians have absolute authority in matters surrounding the Emperor's safety, but no other agency is required to defer to them in any other situation.
